Monthly Membership Plan Details

THE BASICS

Your monthly membership includes unlimited access to all of our cloud-based psychiatric medical services. Memberships are managed by our billing partner Hint Health and contracts are month-to-month. You can terminate anytime with written or verbal notice. If you have any days left in your contract you can still receive services for the remaining days. Membership fees are non-refundable.  

There are no fees to cancel, change, or reschedule an appointment. Patients with a delinquent account may become inactive after five (5) business days. We accept all major credit cards and ACH bank debits via Stripe, using advanced encryption and bank-level security.

INITIAL EVALUATION

Every patient starts off with a face-to-face intake visit which takes place online. You will have a chance to review your medical and psychiatric history with your provider, and determine a treatment plan designed exclusively for you. A pre-intake questionnaire may be administered online.

FOLLOW-UPS

Patients receive UNLIMITED follow-up care. They can take place online, on the phone, by email, or through secure messaging. Sometimes check-ups will be very frequent, if needed, and other times they may be spread apart. Patients may request as many check-up visits as they need. Medication management is the primary service offered and psychotherapy may not be included. A typical visit may last 5-15 minutes per day.

PSYCHOTHERAPY

Our focus is medication management and traditional psychotherapy is generally not included. Brief psychotherapy is included with every visit. If your treatment goal is routine psychotherapy without medication management, this is not the right service for you. A referral can be provided.

MEDICATION REFILLS

Refills may be requested anytime via DreamCloud app, email, text, or phone. We send all prescriptions electronically. In many cases, a follow-up may not be required. However, several medications require monthly follow-ups. CONTROLLED MEDICATIONS MAY ONLY BE REFILLED DURING NORMAL BUSINESS HOURS.

CONTROLLED RX REFILLS

Many psychiatric medications are controlled by the DEA due to the prevalence of misuse and potential dependency. When taken as directed, these medications are therapeutic and safe for use. Any patient who is suspected of misuse or non-compliance is subject to immediate termination and may not be allowed back. We monitor each patient’s medication history on an ongoing basis to ensure safety and compliance.

LABWORK

Our preferred lab partner is Quest Diagnostics, with costs covered by most insurances. For those without insurance, DreamCloud offers affordable low-cost lab orders for all patients. Some patients may require lab work, such as an electrocardiogram (EKG), urinalysis, or other diagnostic tests. Your provider will inform you if lab work or testing is necessary.

NIGHT/WEEKEND POLICY

Patients may contact the office or their provider 24 hours, 7 days a week. We encourage patients to communicate at any time. Calls or messages sent outside of business hours will be returned on the next business day. General appointment requests and medication questions are acceptable. For medical or psychiatric emergencies, patients must call 911 or go to the nearest emergency medical facility.

GENESIGHT™ DNA TESTING

Pharmacogenomic DNA testing can help determine the most effective and compatible medications for you based on your genetic profile. Included in your membership is Genesight testing, developed by Mayo Clinic. A test kit will be sent via FedEx for sample collection at home by yourself or online with your provider. The test itself is free, but Genesight may charge a copay between $0-$330 to process the results, depending on your insurance.

CONFIDENTIALTY

All content from evaluation and visits are completely confidential and will not be shared with any third party without your expressed permission. There are certain circumstances where we are required by law to disclose confidential content if imminent danger is determined to be present which may put you or another person at serious risk for harm. Child or elder abuse is reportable by law.

TRAVEL RX MANAGEMENT

Based in Miami and New York City, our patients come from around the world and travel frequently to other states and even outside the country. Continuing your medication without interruption is very important. Please let your provider know before you travel so that special arrangements can be made around your trip. We can send prescriptions to over 70,000 pharmacies nationwide for Florida and New York residents.

MEDICAL RECORDS/DOCUMENTATION

Copies of medical records are $3/per page. Individualized notes, letters, accommodation requests, special forms, disability determination, and other legal documents are billed at $300/hour in 20m increments. Documentation for emotional support animals is included, if they are prescribed.
