COLLECTING and USING PERSONAL INFORMATION

It is our policy to respect your privacy regarding any information we may collect while operating our website. Accordingly, we have developed this privacy policy in order for you to understand how we collect, use, communicate, disclose and otherwise make use of personal information. We have outlined our privacy policy below.
When you visit our website or use our mobile app, and when you communicate with us via our site, we may collect personal information about you. By "personal information," we mean information that is unique to an individual and by which the individual may be identified, such as name, address, social security number, or health or financial information that is associated directly with an individual. This information is collected in order to provide services to you and to continue to improve and enhance our service offerings. This information may include, but is not limited to, that which you provide voluntarily through your interactions with the site, as well as information about how you use and navigate through the site.
We will not sell, license or transfer personal information we gather from you outside of EHL Psychiatry LLC and any affiliated companies, except as necessary to provide you with information, services or products that we offer independently or jointly with others, as required by law or in connection with judicial or governmental proceedings, or in connection with the sale of all or substantially all of EHL Psychiatry LLC assets or of the assets of a division of DreamCloud.
We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned. Before or at the time of collecting personal information, we will identify the purposes for which information is being collected. We will collect and use personal information solely for fulfilling those purposes specified by us and for other ancillary purposes, unless we obtain the consent of the individual concerned or as required by law.
Personal data may be shared with authorities if significant risk of danger or harm is determined by the provider. Examples include evidence of intent to harm oneself, intent to harm another person, and suspicion of child or elder abuse. These circumstances are considered limits to confidentiality and information is required to be reported by law.
CONFIDENTIALITY of PERSONAL INFORMATION
EHL Psychiatry LLC uses reasonable efforts to protect the privacy and confidentiality of your personal information, including encrypting personal information as it is transmitted between your computer and EHL Psychiatry LLC via our website. In addition, employees of EHL Psychiatry LLC are subject to confidentiality and compliance policies relating to personal information.
When using the DreamCloud website or mobile app, you may be provided with a user identification code (User ID) and a password. Your password is your private key to access your account and personal information. To maintain the confidentiality of your account, you should never share your password with anyone. In addition, you should change it periodically and you should select a password that is difficult for others to identify.
NON-PERSONAL INFORMATION
We may use and disclose to others information about you and your use of the website by which you are not identified, such as the type of browser you use, the files you request, the URL (Uniform Resource Locator) and geographic location from which you request information, the URL that you came from and go to and similar information, and information compiled in the form of statistical, demographic, or aggregated information about visitors generally to the website. This information is referred to in this document as "Non-Personal Information."
We utilize technical mechanisms such as cookies and pixel tags (also called clear gifs) to gather non-personal site usage information. Cookies are small pieces of information that are stored by your browser on your computer's hard drive. Most web browsers automatically accept cookies, but you can usually change your browser to prevent the use of cookies. If you turn off cookies in your browser, however, some portions of our site may not be available to you. Pixel tags are imbedded in the web page and note visits to certain site locations. MOBILE PRIVACY
We record your phone number when you send it to us; ask us to remember it; or make a call or send a text message or SMS to or from DreamCloud.
Most of the other information we collect for mobile, such as your device and hardware IDs and device type, the request type, your carrier, your carrier user ID, the content of your request, and basic usage stats about your device and use of EHL Psychiatry LLC’s products and services does not by itself identify you to EHL Psychiatry LLC, though it may be unique or consist of or contain information that you consider personal.
If you use location-enabled products and services, such as Google Maps for mobile, you may be sending us location information. This information may reveal your actual location, such as GPS data, or it may not, such as when you submit a partial address to look at a map of the area.
Certain use of our products and services allow you to download and/or personalize the content you receive from us. For these products and services, we will record information about your downloads and preferences, along with any information you provide yourself. If the product or service requires you to log in with a DreamCloud account, this information will be associated with your DreamCloud account.
All requests must be sent through your mobile carrier's network and your carrier may have access to it. For information regarding your carrier's treatment of your information, please consult your carrier's privacy policies.
We may share your information with certain third parties we use to perform certain functions, such as billing and text message or SMS delivery. These third parties will be required to treat your information in accordance with the applicable EHL Psychiatry LLC privacy policies. We may also share some information with your wireless operator in order to perform customer service functions.
LINKS TO OTHER SITES
DreamCloud may provide links to other websites, apps, or applets within its website or mobile app. Please be aware that once you leave our website or mobile app and enter another website or mobile app, you may be subject to different privacy practices and policies over which EHL Psychiatry LLC has no control and for which EHL Psychiatry LLC is not responsible. Please use prudence in utilizing such links and in disclosing any sensitive personal health information (PHI) on those sites.
PATIENT COMMUNICATION and PRIVACY
DreamCloud uses multiple channels, including phone, text, email, voicemail, and mobile apps, to communicate with patients regarding both health and non-health related information. The goal of such communication may be administrative, such as providing general information about our services, or to arrange and/or deliver clinical patient care. While every reasonable effort will be made to ensure patient privacy, certain channels may not meet local and federal healthcare privacy standards and we caution against their use. When you communicate with DreamCloud using one of these channels (e.g., requesting an appointment using a social media platform, or requesting refills using a messaging app, etc.), you give your expressed consent to receive responsive communication from DreamCloud using the same channel. We reserve the right to limit such communication at any time. For your safety, we provide a HIPAA-compliant email and telemedicine platform with integrated chat feature which serve as primary channels to exchange private and/or sensitive personal health information with your provider.
AMENDMENTS TO PRIVACY POLICY
We are committed to conducting our business in accordance with these principles in order to ensure that the confidentiality of personal information is protected and maintained. This statement is not intended to and does not create any contractual or other legal rights in or on behalf of any party. EHL Psychiatry LLC may change this privacy policy from time to time at EHL Psychiatry LLC's sole discretion.
